I understand that my group is responsible for ordering any tables, chairs, trash cans, etc. needed for our event from the Physical Plant. I understand that filling out this form does NOT guarantee usage of the desired space. I understand that I will receive a confirmation if and when this request is approved. I understand that if I use any of these facilities and play music, the music played must be radio edit. If non-radio edit music is played, the event will be shut down. I understand I will receive only one warning for misuse of any facility. After that, the organization may be suspended from use of that facility for up to one year.
